Panchmukhi Hanuman - The Indian epic of Ramayana mentions about this form of Hanuman known as Five-faced or Panchmukhi Hanuman. This form was assumed by Hanuman in order kill one of the demon, Ahiravana. He was brother of Ravana, the king of demons and had taken Lord Rama and his brother Lakshman as captives. To kill Ahiravana one was required to extinguish five lamps burning in 5 different directions, at the same moment. Hanuman with his assumed form did it and saved Lord Rama and Lakshman. Starting from right hand side the first one is Lord Hayagriva (The Horse faced). The second one is Lord Narasimha (Lion Faced). Third one is Hanuman himself (Monkey Faced). Fourth one is Lord Garuda (Eagle faced) and the extreme left and the fifth one is Lord Adivaraha (Boar faced).
